AI Utilization Disclosure Addendum
This AI Utilization Disclosure Addendum (this “Addendum”), dated as of [ ] (the “Addendum Date”), is between [[[ ] (the business name) located at [ ] (“Provider”) and FamilySearch International, a non-profit corporation registered in the State of Utah, U.S.A., located at 50 East North Temple Street, Salt Lake City, Utah 84150 (“FSI”). Moreover, this Addendum is an addendum to that certain FamilySearch International Solutions Provider Agreement that was entered into by and between Provider and FSI (the “Agreement”).
In accordance with Section 22 of the Agreement, Provider may not provide to FSI any content created or modified, in whole or in part, using generative artificial intelligence, machine learning, deep learning, or any other form of artificial intelligence (“AI”) unless Provider has provided prior written notice to FSI, and notified its end users so as to provide a full disclosure of what the AI is and how it was or may be used. Provided that Provider gives complete and accurate information for each of the fields in the table below, this Addendum serves as written notice to FSI that Provider is providing to FSI content created or modified using AI.

Provider represents and warrants to FSI that it will provide all legally required information to any of its end users who may download, view, or otherwise interact with AI or content created using AI while accessing Provider’s Solution, and that Provider will further comply with applicable law in all other respects.
Except as described in this Addendum (or an additional AI Utilization Disclosure Addendum), Provider must not provide FSI (or Provider’s end users) with any other content created or modified using AI. Provider bears full responsibility and liability for its use of AI and any content or Solution created or modified using AI.
In any case, Provider must not use any of FSI’s data, materials, confidential information, or other content of any kind (“FSI Content”) in connection with AI (including by providing FSI Content as input to or training/fine-tuning data for AI) without FSI’s express prior written permission. This Addendum does not constitute such permission.
If there is any conflict between the Agreement and this Addendum, the Agreement will govern with respect to the conflict.
The parties hereto have executed this Addendum to be effective as of the Addendum Date.
